<p>The mesothelium is a membrane that completely covers and protects each of the internal organs of the body. This membrane is made up of two layers of cells, one layer surrounds the organ while the other forms a sac around it. The mesothelium produces lubricating fluid which is released between these layers, allowing the moving organs such as the heart and lungs to glide easily against adjacent surfaces.</p>

<p>Mesothelioma is a form of cancer that attacks the mesothelium membrane, and is caused by earlier exposure to asbestos particles. When mesothelioma strikes, cells of the mesothelium become abnormal and divide without control, and malignant cells develop in the mesothelium which can metastasize from their original site to invade and damage nearby tissues and organs within the body. <p>How common is mesothelioma?</p>
<p>Mesothelioma is a relatively rare form of cancer, even though reported incidence figures show a steady increase over the past 20 years or so. Each year approximately 2,000 new cases of mesothelioma are diagnosed in the United States alone. Mesothelioma affects men more so than women and the risk increases with age, but mesothelioma can occur in either sex and at any age.</p>
<p>What are the causes of mesothelioma?</p>
<p>At least 90% of mesotheliomas can be linked to exposure to asbestos, although usually the exposure will have taken place many years before the cancer becomes apparent. It is known that even very low levels of exposure to asbestos can lead to the development of mesothelioma, so it is possible that some patients may not have had any known contact with asbestos but may still have breathed in asbestos fibres without realizing it. Exposure to certain chemicals and radiation has also been linked to mesothelioma, and a small number of mesotheliomas have been linked to a type of radiation called thorium dioxide(thorotast). This was used in some x-rays up until the 1950&#8242;s. There is no evidence linking mesotheliomas with cigarette smoking</p>
<p>How is mesothelioma diagnosed? </p>
<p>The symptoms of mesothelioma are often difficult to diagnose being very similar to the symptoms displayed in other conditions including asbestosis. The diagnosis of mesothelioma will require a review of the patient&#8217;s medical history, including past history of asbestos exposure. A complete physical examination will be performed and x-rays of the chest and abdomen obtained. A CT scan or MRI may also prove useful in the diagnosis of mesothelioma.</p>
<p>Can mesothelioma be treated? </p>
<p>Treatment for mesothelioma varies depending on the location of the cancer, to what extent the mesothelioma has developed, and the patient&#8217;s age and general condition. The options for mesothelioma treatment include surgery, radiation therapy, and chemotherapy.</p>
<p>Is there a cure for mesothelioma?</p>
<p>The only treatment offering any hope of a cure for mesothelioma is an operation to remove the tumour surgically. Unfortunately because mesothelioma is so difficult to diagnose, it is often too far advanced for surgery to be carried out safely. </p>

<p>As with any cancer, treatment for Mesothelioma depends on the location of the cancer, the progression of the disease, the age as well as state of health of the patient.</p>
<p>It is the oncologist who specializes in the disease who will decide on the line of treatment. The disease is difficult to treat and in most cases the prognosis is poor. Treatment options include:</p>
<p>1.  If the diagnosis is done in the early stages surgery may be recommended to remove all cancerous tissues. This means thoracoscopy, VATS or video assisted thoracic surgery, mediastinoscopy (used for staging), or laproscopy.  Often, doctors will advise palliative procedures like chest tube drainage and pleurodesis, thoracoscopy and pleurodesis, pleuroperitonial shunt, or pleurectomy, which treat the symptoms of mesothelioma rather than the disease.</p>
<p>2. </p>
<p> Radiation is prescribed aggressively for mesothelioma patients and is often given in combination with surgery or in order to control symptoms, palliatively. Research on using radiation therapy using implants or UV light therapy is in progress as traditional radiation therapy damages surrounding healthy tissue.</p>
<p>3.  Chemotherapy is an option and around 12-20% of patients respond to the drugs. Anti-cancer drugs destroy cancer cells and prevent their spread. In mesothelioma, chemotherapy is not considered to be curative. The aim is to prevent spread of the disease; shrink the tumor prior to surgery, known as neoadjuvant therapy; to annihilate any remains of the tumor in the body post surgery; and to relieve pain and other discomfort, palliative chemotherapy. Experts recommend prescribing premetrexed along with cisplastin. These drugs have shown positive results and this is now standard care for Mesothelioma not treatable by surgery.</p>
<p>4.  Biological therapy using interleukin 2.</p>
<p>5.  Immunotherapy where the body&#8217;s own immune system is stimulated to fight the disease. Biological response modifiers, BRMs, are used in treatment. These enhance the functions of the immune system; regulate metabolic reactions that promote growth of cancers; alter cancer cell division; block or reverse processes that lead to formation of cancers; and prevent spread of cancer.</p>
<p>6.  Gene therapy, where attempts are made to correct the gene sequence that causes the cancer. This is of two kinds, replacement gene therapy and knockout gene therapy.  </p>
<p>7.  Administration of lovastatin, a cholesterol lowering drug.</p>
<p>8.  Photodynamic treatment where a photosensitive medication is activated using a laser. This is done during the surgical removal of cancerous tissues.</p>
<p>9.  Draining of fluid in the chest or abdominal cavity followed by introduction of medications into the cavity to prevent fluid accumulation is done for patients who have uncontrolled fluid accumulation and intense discomfort stemming from it.</p>
<p>10.  Combination therapy or Trimodal therapy which means surgery accompanied by radiation and chemotherapy.  </p>
<p>Often, complementary medicines are also used. Termed to be holistic this kind of treatment focuses on a patient&#8217;s physical, mental, emotional, and spiritual well being.</p>
<p>In the case of Mesothelioma which is untreatable one can opt for clinical trials of new treatments that are in progress in several research laboratories and centers. The best advice on the line of treatment would be that recommended by the oncologist or physician. </p>

Of the mesotheliomas, 55 of 61 (90%) stained for thrombomodulin, 56 of 61 (92%) for cytokeratin 5/6, 47 of 51 cases (92%) were positive for calretinin, and 39 of 43 (91%) were positive for CD44H. Of the metastatic adenocarcinomas, 12 of 63 (19%) cases were positive for thrombomodulin, 9 of 63 (14%) were positive for CK5/6, and 27 of 60 (45%) were positive for CD44H. With calretinin, only 1 case of 59 (2%) showed positive nuclear staining. All four antibodies stained reactive mesothelium; thrombomodulin also stained endothelium; and CD44H variably stained lymphocytes, macrophages, and fibroblasts. We conclude that all four antibodies show high sensitivity for epithelioid mesothelioma, but only calretinin (98%), cytokeratin 5/6 (86%), and thrombomodulin (81%) show sufficient specificity for practical use in this situation.</p>
<p>Another interesting study is called, Surgical treatment of pleural Mesothelioma by PM McCormack, F Nagasaki, BS Hilaris and N Martini &#8211; The Journal of Thoracic and Cardiovascular Surgery, Vol 84, 834-842. Here is an excerpt: From 1939 through 1981, 170 patients were seen and treated for pleural mesothelioma. Twenty-one tumors were benign, 47 were fibrosarcomatous, and 102 were epithelioma. Resection was the main mode of treatment in benign and fibrosarcomatous mesothelioma. Treatment of diffuse epithelial mesothelioma presented the greatest challenge. Surgical therapy, radiation therapy, and chemotherapy were used in combination in these patients. The review of our patients treated prior to 1972 had shown no benefit from including pulmonary resection in the surgical treatment of these tumors. Since then, all patients with diffuse mesothelioma were treated by pleurectomy without pulmonary resection. Both internal and external radiation therapy were also used to enhance local control. Forty-nine percent of patients with epithelial mesothelioma lived 1 year. The median survival in patients whose disease was controlled by these methods was 21 months. Despite the poor prognosis in malignant mesothelioma, better controlled by these methods was 21 months. .</p>
<p>Another interesting study is called, RB protein status and clinical correlation from 171 cell lines representing lung cancer, extrapulmonary small cell carcinoma, and mesothelioma. By Shimizu E, Coxon A, Otterson GA, Steinberg SM, Kratzke RA, Kim YW, Fedorko J, Oie H, Johnson BE, Mulshine JL, et al. &#8211; National Cancer Institute-Navy Oncology Branch, National Cancer Institute, Bethesda, Maryland 20889. &#8211; Oncogene. 1994 Sep;9(9):2441-8. Here is an excerpt: Abstract &#8211; We have studied RB protein expression in 171 cell lines derived from patients with small cell lung cancer (SCLC), non-small cell lung cancer (NSCLC), pulmonary carcinoid, mesothelioma, and extrapulmonary small cell cancer (EPSC) and have correlated this data with clinical outcome. We detected absent or aberrant RB protein expression in 66/75 SCLC, 12/80 NSCLC, 1/6 carcinoid, 0/5 mesothelioma, and 4/5 EPSC samples. In addition, we observed integration of human papilloma virus (HPV) DNA in the single EPSC cell line that retained wildtype RB protein. We did not detect integration of HPV, SV40 or adenoviral DNA in other tumor samples with wildtype RB status. We also noted a stable, hypophosphorylated mutant RB in 12 SCLC and 3 NSCLC samples which might have been falsely interpreted as wildtype by current immunohistochemical techniques. Analysis of the matched clinical data showed no associations between RB status and age, sex, extent of disease, performance status, smoking history, and previous treatment. In addition, retrospective analyses showed no consistent correlation of RB protein expression with either best clinical response, overall survival, or in vitro chemotherapeutic drug sensitivity. The stable expression of RB after gene transfection into RB(-) SCLC cells, however, resulted in a trend toward increased in vitro resistance to etoposide, cisplatin and doxorubicin.</p>

<p>Mesothelioma is a deadly cancer that is directly linked to asbestos exposure. By the time this cancer is diagnosed the disease is usually well advanced. If diagnosed with Mesothelioma, it is imperative to become informed about the different forms of Mesothelioma treatment due to the aggressive form of this disease.</p>
<p>Mesothelioma attacks the mesothelium or the protective lining that covers and protects many of the body&#8217;s internal organs. One form of this deadly cancer is Peritoneal Mesothelioma. It invades the peritoneum or the membranous lining that houses the stomach, liver, spleen, pancreas and intestines.</p>
<p>Peritoneal Mesothelioma accounts for less than a quarter of all Mesothelioma cases. It is the second most common form of Mesothelioma after Pleural Mesothelioma. Symptoms of Peritoneal Mesothelioma include, but are not limited to, abdominal pain, weakness, loss of appetite, weight loss, abdominal swelling and bowel obstruction. </p>
<p>In order for proper Mesothelioma treatment to begin, several factors must first be taken into consideration. Details such as the extent of the cancer, the patient&#8217;s overall health, past medical history and age, should be evaluated. These details need to be taken into account to determine the best Mesothelioma treatment for an individual diagnosed with Peritoneal Mesothelioma.</p>
<p>The most common forms of treatment for an individual diagnosed with Peritoneal Mesothelioma are surgery, radiation, chemotherapy and palliative therapies. These can be given either in tablet form or intravenously. The chemotherapy drugs enter the blood stream and kill off cancerous cells as it makes its way through the body.</p>
<p>Palliative therapy is also used as a form of Mesothelioma treatment. Ascites, or fluid accumulation, often takes place in the abdomen when an individual suffers from Peritoneal Mesothelioma. To relieve pressure and to make the patient more comfortable fluid is extracted by use of needles and suction. </p>
<p>Once your diagnosis is confirmed and you have begun treatment, it is imperative that you seek the advice of an attorney who specializes in asbestos-related illness. <p>Mesothelioma cancer is considered to be the rarest and deadliest types of cancer, and usually affects the mesothelial tissues of the body organs, normally abdomen and lungs. It develops malignant and cancerous cells in mesothelium, which is the protective cell covering many internal organs of the body. It can even damage other body organs by spreading out the damaged cells to them.</p>
<p>The main cause of this type of cancer in about 50-80% cases is exposure to the asbestos. People involved in occupations like constructions jobs in insulators, shipyards and boiler makers are at a risk of catching asbestos disease. It is a very restrained form of cancer which provides only few symptoms that can be noticed until the disease becomes enormously advanced.</p>
<p>Pleural mesothelioma is the most usual type of the cancer, indicated by chronic coughing, shortness of breath in combination with common cold and allergies. Other pleural mesothelioma symptoms include chest pain, coughing blood, fatigue, wheezing, swollen lymph node, loss of appetite, loss of weight and lung infection.</p>
<p>Peritoneal mesothelioma is another form of the cancer, which generally affects the covering around the intestines and the stomach. Symptoms of this type of cancer include weight loss, fever, anaemia, pain in abdomen, swelling in abdomen or bowel obstructions. Another type of the cancer is pericardial mesothelioma, the symptoms of which including palpitations, persistent coughing, chest pain and shortness of breath.</p>
<p>It has been estimated that about 2000 new cases of this type of cancer are diagnosed every year. It is to be noted that only a professional cancer expert can diagnose mesothelioma. The disease starts in small lumps in the pleura membrane, that may not be scanned on X-rays, until they are advanced and grown up in large size. The symptoms of the disease may not come into view until 30-50 years of exposure to asbestos.</p>
<p>The treatment options available for this type of cancer are:</p>
<p>Chemotherapy: Chemotherapy with doxorubicin or adriamycin has been 20% responsive in cases of mesothelioma.</p>
<p>Radiation Therapy: This disease is quite moderately responsive towards radiation therapy also. It can improve the quality of life of the patient, as it can reduce shortness of breath and pain in the body, while it can not cure the problem.</p>
<p>Surgery: Surgical choices like removing the cancer tumour by cutting out the lun or decortication have also not been very successful so far.</p>
<p>Photodynamic therapy: In this process, the cancer cells are given an intravenous photosynthesizer, which are afterwards activated in order to destroy the cells with the help of a laser light of specific wave length. This therapy is still in its experimental stages.</p>
<p>The average survival time of a patient from the time of observance of the cancer is about 4-24 months, however, with proper care, some patients have also been seen living for many years. Therefore, it is advisable for those who are working in asbestos prone localities to regularly consult a doctor and go for regular check ups, so that any possibility for the cancer can be diagnosed at an earlier stage.
